Crusader Community Health Holds Annual Celebration "Fall in Love with Caring"

Crusader Community Health is hosting their annual fundraiser, “Fall in Love with Caring”, on Friday October 24th, 6-10 p.m., at Giovanni’s, 610 North Bell School Road, Rockford. Cost of the event is $100 or $1,000 for a table of ten. The evening includes music by the Stellanova’s, an autumn themed dinner, announcement of the “Spirit of Caring” awards, and a fantastic silent and live auction.

The live auction will feature a one week stay at a Marco Island, Florida, condo donated by Chuck & Sue Thomas; an exquisite gourmet dinner for 6 prepared by Giovanni’s executive chef donated by Giovanni’s; Wisconsin Dells weekend package including a two night stay for 8 persons at the Kalahari Waterpark Resort; a 5 night stay at the Island Penthouse on Olde Marco Island, Florida donated by Pete & Jennifer Kruchten; a children’s golf cart donated by Lexus of Rockford and a deluxe grill donated by Benson Stone Company. A listing of silent and live auction items will be available on the website (www.crusaderhealth.org) starting October 1st and will be updated weekly.

The raffle is a chance to win the grand prize, a 50 inch Panasonic Plasma TV and Blu-ray disc player donated by Riverside Community Bank; second prize is an IBM ThinkPad Laptop Computer donated by Rockford Information Technologies; the third prize is $1,000 cash donated by Crusaders Health Foundation Board members. Raffle tickets sell for $5 each or 5 for $20 and can be purchased online at www.crusaderhealth.org, click on Crusaders Health Foundation to get the drop down menu then click on “Fall in Love with Caring.”

Proceeds from the event will benefit the implementation of an electronic health records system and other needs at Crusader Community Health as designated by the Crusaders Health Foundation Board of Directors. For the past 36 years, Crusader Community Health has strived to ensure that all persons in our community have access to quality, affordable primary health care. During the past 12 months the clinic served the health care needs of nearly 40,125 individual patients.

Crusader Community Health Pharmacy

A full-service pharmacy is available within each Rockford location and a medication dispensary is available at the Bevidere location. 

The Crusader Community Health pharmacy offers patients access to low cost prescription medications.  A medication subsidy coordinator is available to assist eligible patients obtain prescribed medications at a low cost through a variety of programs.

The Pharmacies at the West State and Broadway locations are open for service during the following hours:

Fees

As a Federally Qualified Health Center (FQHC), Crusader Community Health offers discounts based on federally established guidelines.  You may be eligible for a discount if you are not covered by an insurance plan. 

Discounts are based upon family size and income.  Proof of income is required to be considered for a discount.  Discount levels are 25%, 50%, 75% and 100% on eligible services.  There is a minimum $20 fee due per medical visit and a minimum $30 fee due per dental, optical, or podiatry visit, regardless of discount.  Discounts are re-evaluated annually.

Patients may be required to be assessed for eligible entitlements (entitlements may include Medicaid or other public benefits).  Crusader Community Health provides assistance in completing entitlement applications.

Most laboratory and radiology services for self-pay patients ordered and processed at the clinic are eligible for discounts.  Laboratory and radiology services for patients with third-party payers are billed by the clinic's contracted vendors.

Some Crusader Community Health services are ineligible for a discount.  These services include all eyewear and dental procedures involving prosthetics (dentures and partials) and root canals.
